Mission Trip Application

Our goal is to make the application process as simple as possible for all of our mission trip participants. Please email us at international@stchm.org if you have any problems applying.

Four Easy Steps:

STEP 1    Complete the International Mission Trip Application.
The Application may be submitted directly online or you may download, complete and print the form, and then mail to:

You should receive preliminary confirmation of acceptance within ten days.

STEP 2    Download and complete the International Mission Trip Agreement.
Once your trip is confirmed, download and complete this form and have the Agreement notarized. Mail the notarized form with your $200 non-refundable* trip deposit to the address above.

* Trip deposit is non-refundable, except in the event the trip is full or cancelled.
The deposit will be applied towards the cost of your trip.

STEP 3    STCHM will make all air reservations and will e-mail the flight confirmation number to you. You may then purchase the ticket with your own credit card.

STEP 4    Submit the balance of the land cost** along with a clear copy of your passport by the balance due deadline. You will receive a handbook, itinerary and other important information about the trip.

** Trip payments can be paid online with a credit card. However, we request that you add 3% to each payment amount made online to cover the additional fee STCHM is charged for online processing.
